Once completed, kindly email your responses in MS Word format (*.doc / *.docx), together with all the necessary attachments, to the email address found in the application form.

Your answers to these questions will help the SWAP team get to know you and your organization better.
Please answer these questions clearly and concisely. Please carefully review the additional completion guidance that can be found in the application form and supporting templates.
Problem
1. What is the social / environmental problem that your organization is trying to solve? Convince us that this issue is worth investing in and why it matters to your country / region.
Solution / business model
2. Please provide a brief history of the organization – what was the motivation to start the organization, when were you set up, and who were the people involved?
3. How does your organization go about creating the desired change? Please describe the key elements / different steps of your solution / business model. You can include diagrams and pictures as an attachment to the application; these will not be counted towards the word limit.
4. What are the strengths and weaknesses of your organization and where do you see opportunities and threats in the market you operate in. Please prepare a SWOT analysis and attach to the application.

5.a) What is the current status of your organization? Please provide financials and positive impact figures from the past 3 years (use Application templates provided). If not available, please briefly explain why.
5.b) What has been the toughest challenge faced by your organization so far, and how did you solve / get past it?
6. Where do you see your organization in the next 3-5 years? Please be as concrete as possible in terms of specific goals to be achieved – how, and by when?
Team, board, cooperation partners and network
7.a) Enumerate the top 3-5 people on your team. Please give us their names, job titles, role/s in the organization, and previous experience / achievements. Is there any other role you need filled within the organization?
7.b) Does your organization have a board of directors and/or advisors? If yes, who are your board members? If no, simply leave blank.
7.c) Does your organization have other important partners that add value through financial or non-financial means? Who are they and why are they important? e.g. corporations, other non-profits, microfinance partners, government, etc.

8. Have you been part of any previous business plan context or incubation program? If so, when, and by what organization? What financial and non-financial support have you received thus far?
9. What financial support (amount and type, e.g. grant, loan or equity) would you need for the next 1-3 years? What would this be used for?
10. What kind of additional non-financial resources would you need from LGT Venture Philanthropy and its regional partners?

- Have you completed the application form according to the stated guidance?
